Deborah Horton, M.Ed., LCPC, LPSC, NCC has very limited openings currently and is using this prescreening questionnaire to determine if potential clients would be a good fit for any opening she currently has available. Please complete this form and she will contact you to either set up a free consultation appointment or to send you referrals to a provider who will be a better fit.
Deborah is now seeing only adult women ages 19 to 65. She accepts the following insurance only: BCBS Montana, Allegiance, First Choice, EBMS, Aetna, Cigna, and Pacific Source. She does not accept Medicaid. She also accepts private pay clients. She has appointments for weekday mornings, afternoons, and a couple of slots after 4pm. No weekend appointments. She only has every other week appointments available. No online appointments are available.
